Photo Release: Photographs are sometimes taken of Awana ministry activities for publicity and promotional purposes, which include, but are not limited to, in-house presentations, church web site, brochures and newsletters. By signing below, you are granting the church to use photographs of the above mentioned minor(s) as stated.

Medical Release: I the undersigned parent or guardian, hereby consent to my child participating in AWANA Clubs, an event sponsored by First Coast Baptist Church on Sunday evenings from September 2023 - May 2024. I certify that my child is able to participate in all activities. In the event an emergency occurs, and that neither myself or my emergency contact person cannot be reached, I hereby authorize First Coast Baptist Church to make emergency medical decisions for my child. I UNDERSTAND AND HEREBY AGREE TO ASSUME ALL OF THE RISKS WHICH MAY BE ENCOUNTERED ON SAID ACTIVITY, INCLUDING ACTIVITIES PRELIMINARY AND SUBSEQUENT THERETO. I do hereby agree to hold First Coast Baptist Church and its agents and employees, harmless from any and all liability, actions, causes of actions, claims, expenses and damages on account of injury to my child or property, even injury resulting in death, which I now have or which may arise in the future in connection with the activity or participation in any other associated activities.
